A transition plan is a document that outlines the steps that are needed to move from one state or situation to another. It is used to plan and execute the transition of a project from one phase to another, or to move a project to the operational BAU phase.  Transition plans can be used to ensure a smooth and successful transition, and to minimize disruption or downtime during the process.
