A transition plan is a document that outlines the steps that are needed to move from one state or situation to another. It is used to plan and execute the transition of a project from one phase to another, or to move a project to the operational BAU phase.  Transition plans can be used to ensure a smooth and successful transition, and to minimize disruption or downtime during the process. They can be created for a variety of transitions, including the implementation of new systems or processes, the launch of a new product or service, or the transfer of a project to a new team or organization.
